29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about May 29, 2025, employees conducting roofing work on a new residential home located at 1727 N. River Circle in River Hills, WI 53217, with an approximately 7:12 pitch roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 10 feet on the west side of the house.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. (a) On or about May 29, 2025, at a jobsite located at 1727 N. River Circle in River Hills, Wisconsin, the employer did not ensure that all employees installing shingles and water proofing materials were trained to recognize hazards associated with working from heights greater than six feet (~10 feet), therefore exposing employees to fall hazards.
