Occupational Safety and Health Act of 1970, Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a.) On or about June 4, 2025, Employees at 1110 Commons Blvd., Reading PA working near an area where a slow-moving recessed conveyor was located were exposed to trip hazards.  Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.303(g)(2)(ii):  In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:    a) 1110 Commons Blvd., Reading, PA - Shredder Area - A metallic electrical box was exposed to physical damage in that the bottom of the box was separated from the rest of the box, exposing inner electrical conductors, creating a shock hazard, on or about June 4, 2025.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. .

29 CFR  1904.40(a):The employer did not provide an authorized government representative the records within the four business hours.  a) 1110 Commons Blvd., Reading, PA -  On or about June 4, 2025, the employer failed to provide the OSHA Form 300 and 300a Logs, for Calendar Years 2022-2025 within the four business hours of the request.  The forms were not provided to the authorized government representative by the employer until June 11, 2025.  No abatement certification or documentation required.

29 CFR 1910.37(b)(5):  Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  a) 1110 Commons Blvd., Reading, PA ? A door with an exit sign over head had items including, but not limited to, trash cans stored in front of it.  The adjacent door, which was properly identified as an exit, was the intended exit door for employee use.  The door with storage in front of it, which was not intended for employee use, was not identified with ?Not an Exit? signage, on or about June 4, 2025.  ABATEMENT NOTE: Among other methods, feasible and acceptable means of abatement would be to remove the exit sign in its entirety from the door which is not intended as the exit, and to replace the exit sign with ?Not an Exit? signage, while ensuring compliance with 29 CFR 1910 Subpart E.  No abatement certification or documentation required.
