29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 130 North Collingwood Avenue, Syracuse, NY  13206, on the roof, on or about 6/5/25;  Workers were performing roofing work on a 10:12 pitch roof, with a ground to eave height of 12 feet, without a positive means of fall protection.  Melo Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1534773, citation number 1, item number 1a and was affirmed as a final order on 1/28/2022, with respect to a workplace located at 7854 Restmoor Drive, Baldwinsville, NY 13027.  Melo Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1602993, citation number 1, item number 1 and was affirmed as a final order on 11/18/2024, with respect to a workplace located at 3463 Van Wie Drive East, Baldwinsville, NY 13027.  Melo Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1623495, citation number 1 , item number 1 and was affirmed as a final order on 11/18/2024, with respect to a workplace located at 750 West Genesee Street, Syracuse, NY 13204.  Melo Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1625957, citation number 1, item number 2 and was affirmed as a final order on 11/18/2024, with respect to a workplace located at 705 Bailey Road, Syracuse, NY 13212. 29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 130 North Collingwood Avenue, Syracuse, NY  13206, accessing the roof on the south side of the building, on or about 6/5/25;  Workers used an extension ladder where that ladder did not extend at least 3 feet above the upper landing.    b) 130 North Collingwood Avenue, Syracuse, NY  13206, accessing the roof at the front of the building, on or about 6/5/25;  Workers used an extension ladder along the rake, where that ladder did not extend at least 3 feet above the upper landing.
