29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) On June 12, 2025, at the above addressed jobsite, employees engaged in roofing activities were exposed to falls of greater than approximately 10, 20, and 30 feet, without the use of fall protection or a warning line and safety monitoring system. No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On June 12, 2025, at the above addressed jobsite, the portable ladder used by employees to access and egress three levels of the house where roofing work was being performed were not extended to have the side rails extend at least three (3) feet above the upper landing. The employees were exposed to fall hazards of approximately 18 feet when accessing the ladder from ground level to the second level outer courtyard. b) On June 12, 2025, at the above addressed jobsite, the portable ladder used by employees to access and egress three levels of the house where roofing work was being performed were not extended to have the side rails extend at least three (3) feet above the upper landing. The employees were exposed to fall hazards of approximately 12 feet when accessing the ladder from the second level outer courtyard to the third level inner courtyard. c) On June 12, 2025, at the above addressed jobsite, the portable ladder used by employees to access and egress three levels of the house where roofing work was being performed were not extended to have the side rails extend at least three (3) feet above the upper landing. The employees were exposed to fall hazards of approximately 10 and 20 feet when accessing the ladder from the third level inner courtyard to the roof. Reid Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 1926.1053(b)(1), which was contained in OSHA inspection number 1671299, citation number 1, item number 5, and was affirmed as a final order on August 16, 2023, with respect to a workplace located at 1653 West Barry Ave., in Chicago, Illinois. No abatement certification or documentation is required for this item.
