29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a. On or about June 30, 2025, at the above addressed jobsite, an employee was performing demolition operations from a JLG X430AJ aerial lift, approximately (15) feet above the ground, the employee was not wearing a personal fall arrest system, thereby exposing the employee to fall hazards.
