29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. On or about July 2, 2025, employees were exposed to a fall hazard of approximately 20 feet while performing construction activities including sheathing installation because fall protection was not properly utilized and the employer did not ensure that each employee was trained in the proper use and operation of the personal fall arrest systems and other protection used on the jobsite.
