29 CFR  1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  Employees engaged in machining activities at the following pieces of equipment throughout the facility were exposed to struck-by and amputation hazards in that machine guarding were not affixed and secured to the machines: a) Machine center in building #1, the electrical interlock on the door was not provided. The Fryer Milling machine model number VB-60, serial number 60333 did not have any door with interlock safety features. The machine just utilizes some metal panels to slide in and out when operating it. b) Machine center in building #1, the electrical interlock on the door was not provided. The Fryer Milling machine model number MB-10R, serial number 10833 did not have any door with interlock safety features. The machine just utilizes some metal panels to slide in and out when operating it.

29 CFR  1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  Employees engaged in fabrication activities using the Ryobi 8" Pedestal Bench Grinder at the Mechanical Press Brake area in building #2 were exposed to caught-in hazards in that the work rest opening was not maintained at a maximum of one-eight inch.  The work rest was found to be approximately eleven-twelves inch away from the grinding wheel.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch:  Employees engaged in fabrication activities using the Ryobi 8" Pedestal Bench Grinder at the Mechanical Press Brake area in building #2 were exposed to struck-by hazards in that the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1.5 inches. The abrasive wheel bench grinder was not equipped with a proper tongue guard which leaves a big gap.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by rotating parts, and/or flying chips:  Employees engaged in machining activities at the Maintenance Shop in building #2 were exposed to struck-by and amputation hazards in that machine guarding were not provided and utilized. The Dayton 9-Speed 20" Floor Model Drill Press, Model number 3Z919C, the rotating drill chuck was not guarded.  Zephyr Products, Inc. was previously cited for a violation of this occupational safety and health standard 1910.212(a)(1), which was contained in OSHA inspection number 1482255, citation number 1, item number 1, and was affirmed as a final order on 10-02-2020, with respect to a workplace located at 3030 Wilson Avenue Leavenworth, KS 66048.
