29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard, and/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017: (a) Powder coating area; On or about July 24, 2025, and dates prior, the employer did not provide training on applicable topics, including but not limited to, fall hazard recognition and mitigation methods, regarding employees who were working from elevated structure(s) to clean the Viking parts blaster and washing equipment at approximate heights ranging between 11 feet to 14 feet above lower-levels. Employees used personal fall protection equipment, however, they were not effectively protected from striking the ground in the event of a fall.

29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems are rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level: (a) Powder coating area; On or about July 24, 2026, and dates prior, while cleaning the Viking parts blaster and washing equipment at approximate heights ranging between 11 feet to 14 feet above lower-levels, employees were exposed to free falls exceeding 6 feet and potential impact with lower-levels due to the use of a 6-foot shock-absorbing lanyard in conjunction with a 6-foot anchor strap, attached to adjacent guardrail.
