29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  The employer failed to submit documentation of abatement within 10 days of the abatement date for Citation 1 Item 1, and Citation 1 Item 2.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about August 8, 2025, employees reroofing a residential building, at a height greater than six feet, was not protected from falls to the lower level.  Green Star Metal Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.501(b)(13), which was contained in OSHA inspection number 1788087, citation number 2, item number 1A and was affirmed as a final order on January 6, 2025, with respect to a workplace located at1356 Dunning Street, Menasha, WI 54952.  Green Star Metal Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.501(b)(10), which was contained in OSHA inspection number 1558023, citation 1, item number 1 and was affirmed as a final order on March 1, 2022, with respect to a workplace located at 725 Blue Harbor Drive, Sheboygan, WI 53081.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about August 08, 2025, the employer did not ensure employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work in that all employees were working from heights greater than six feet without fall protection.  Green Star Metal Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.503(a)(1), which was contained in OSHA inspection number 1788087, citation number 2, item number 1B and was affirmed as a final order on January 6, 2025, with respect to a workplace located at 1356 Dunning Street, Menasha, WI 54952.
