A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. (a) On August 13, 2025, at or near the intersection of 11th Ave S and Westward Drive, Grand Forks, ND, an employee installing water lines in a trench 8 feet in depth, was not provided with safe means of egress. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1672454, Citation 1, Item 1a, and was affirmed as a final order on March 6, 2024, with respect to a workplace located at 1913 Gateway Drive, Grand Forks, ND 58203.

(a) On August 18, 2025, at or near the intersection of 11th Ave S and Westward Drive, Grand Forks, ND, an employee was taking measurements in a trench where no tabulated data was onsite or available offsite for the 8.5' x24' trench box. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1672454, Citation 3, Item 2, and was affirmed as a final order on March 6, 2024, with respect to a workplace located at 1913 Gateway Drive, Grand Forks, ND 58203.
