29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about August 14, 2025, the employer, Welsh Excavation Company, Inc., failed to conduct frequent and regular inspections of the job site to identify and correct deficiencies in the traffic control setup. Employees were performing trenching operations in the eastbound lanes of Montgomery Road (State Route 25), where traffic was redirected using channelizing barrels. The spacing of barrels was inconsistent and exceeded recommended distances under Part 6 of the Manual on Uniform Traffic Control Devices (MUTCD) for taper channelization, including gaps greater than 25 feet in a 25 mph zone. As a result, employees were exposed to struck-by and crushed-by hazards from road users, including an incident where a vehicle entered the work zone and partially fell into an open trench
