29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  On or about August 18, 2025, employees were exposed to fall and electrical hazards and the employer failed to ensure the work area was inspected by a competent person:  a. An employee was working from a scissor lift near live electrical busbars and bus box and the employer did not inspect the area to ensure the equipment was de-energized or effectively guarded.  b. Employees installing the new paint line equipment were exposed to fall hazards near the adjacent parts washer and the employer did not inspect the walking working surfaces such that floor holes were effectively protected.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about August 18, 2025, employees were exposed to fall and electrical hazards and the employer failed to ensure employees were aware of the hazards and methods to eliminate or control them:  a. An employee was working from a scissor lift near live electrical busbars and a bus box and the employer did not ensure the employee was trained to recognize and avoid the hazard.  b. An employee installing new paint line equipment was exposed to unprotected floor holes around the adjacent parts washer and the employer did not ensure the employee was trained to recognize and avoid the hazard.
