29 CFR  1926.102(a)(1):  The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At a commercial worksite located at 4101 E 105th Terr , Kansas City, Missouri- An employee was exposed to serious eye injury while using a Hitachi brand pneumatic nail gun without eye protection

29 CFR  1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial worksite located at 4101 E 105th Terr, Kansas City, MO. Employees were exposed to serious injuries, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approx. 13 to 17 ft fall hazard to the concrete surfaces below.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a commercial worksite located at 4101 E 105th Terr, Kansas City, MO. Employees were accessing a working surface on a  roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.
