29 CFR 1910.28(b)(6)(ii): The employer did not ensure that each employee 4 feet (1.2 m) or more above dangerous equipment must be protected from falling by one of the systems listed in 29 CFR 1910.28(b)(6)(ii)(A)-(D): On or about August 29, 2025, employees working with the cardboard compactor were exposed to fall hazards of approximately 6.5 feet into the compactor and the employer did not provide appropriate fall protection.
