29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about September 25, 2025, at a construction site located 917 East Trailside court, Oak Creek, WI 53154, the employer did not perform frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected.  Residential Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 1832515, citation number 1, item number 1 and was affirmed as a final order on September 4, 2025, with respect to a workplace located at 325 Sandstone Street, Slinger, WI 53086.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about September 25, 2025, at a construction site located 917 East Trailside court, Oak Creek, WI 53154, employees were installing shingles on the west side of the first story of a residential roof with a 10:12 pitch, were not tied off. This exposed employees to a fall hazard of approximately 13 feet from roof to the ground below.  (b) On or about September 25, 2025, at a construction site located 917 East Trailside court, Oak Creek, WI 53154, an employee was installing shingles on the north side of the first story of a residential roof with a 10:12 pitch, was not tied off. This exposed the employee to a fall hazard of approximately 12 feet from roof to the ground below.  (c) On or about September 25, 2025, at a construction site located 917 East Trailside court, Oak Creek, WI 53154, an employee was installing shingles on the apex of the first story of a residential roof with a 10:12 pitch, was not tied off. This exposed the employee to a fall hazard of approximately 20 feet from roof to the ground below.  Residential Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1832515, citation number 2, item number 1 and was affirmed as a final order on September 4, 2025, with respect to a workplace located at 325 Sandstone Street, Slinger, WI 53086.  Residential Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1669052, citation number 1, item number 1, group letter a, and was affirmed as a final order on June 24, 2023, with respect to a workplace located at 827 Marymere Drive, Oconomowoc, WI 53066.  Residential Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1572879, citation number 1, item number 1 and was affirmed as a final order on May 2, 2022, with respect to a workplace located at 231 Four Winds Court, Hartland, WI 53029.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about September 25, 2025, at a construction site located 917 East Trailside court, Oak Creek, WI 53154, the portable ladder(s) being used by employees to access the roof of the structure did not extend at least three feet above the upper point of contact with the eave, nor were ladders secured in place. This exposed employees accessing and egressing the roof, to a fall hazard onto the ground below.  Residential Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1832515, citation number 2, item number 2 and was affirmed as a final order on September 4, 2025, with respect to a workplace located at 325 Sandstone Street, Slinger, WI 53086.  Residential Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1669052, citation number 1, item number 1b, and was affirmed as a final order on June 24, 2023, with respect to a workplace located at 827 Marymere Drive, Oconomowoc, WI 53066.
