29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   JC Ford Gen 2 Sheeter (Serial PS-150706; Model TC-1246-6) Back portion of Line 2:  On or about September 25th, 2025, sanitation workers were exposed to laceration and amputation injuries from ingoing nip points between the rear roller and the front roller of the sheeter while cleaning the equipment.  The employer failed to ensure that employees implemented energy control application steps prior to allowing them to engage in servicing and maintenance work, such as cleaning and sanitation activities.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.  JC Ford Gen 2 Sheeter (Serial PS-150706;Model TC-1246-6) Back portion of Line 2:  On or about September 25th, 2025, sanitation workers were exposed to laceration and amputation injuries from ingoing nip points between the rear roller and the front roller of the sheeter while cleaning the equipment.  The employer did not ensure that each authorized employee affixed a personal lockout or tagout device prior to allowing them to engage in servicing and maintenance work, such as cleaning and sanitation activities.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  JC Ford Gen 2 Sheeter (Serial PS-150706;Model TC-1246-6) Back portion of Line 2:  On or about September 25th, 2025, sanitation workers were exposed to laceration and amputation injuries from ingoing nip points between the rear roller and the front roller of the sheeter while cleaning the equipment.  The employees were not adequately trained on the means and methods needed to effectively isolate and control hazardous energy sources.  Employees were exposed to the potential for unexpected start-up, energization, and/or release of stored energy which results in amputation/crushing while cleaning the rollers.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

JC Ford Gen 2 Sheeter (Serial PS-150706;Model TC-1246-6) Back portion of Line 2:  On or about September 25, 2025, the employer did not ensure that periodic inspections of energy control procedures were conducted at least annually to identify and correct any deviations or inadequacies.  Sanitation workers were exposed to laceration and amputation injuries from ingoing nip points between the rear roller and the front roller of the sheeter while cleaning the rollers without first locking and tagging the equipment out.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
