29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Plant 28, Masa Department 208- On or about November 28, 2025, employees were exposed to machine hazards from moving parts while unclogging flour from Cyclone No.7. After three employees reached inside the cyclone pipe one at a time, the third employee extended their hand upward, resulting in the amputation of three fingers on the right hand. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.

29 CFR  1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  (a) On or about November 28, 2025, the employer failed to ensure that each of the three employees involved in the servicing task of removing flour accumulated at the base of cyclone #7 affixed a personal lock onto a group lockout device. Neither employee used a lockout device to lockout the cyclone #7, thereby the three employees were exposed to amputation hazards and one employee suffered an amputation to three fingers on the righthand.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a. On or about November 28, 2025, the employer did not contact OSHA within 24 hours of a work-related incident in which an employee sustained an amputation of three fingers on the right hand after coming into contact with the blades of  the Centrifuge Cyclone No.7.
