29 CFR 1910.269(o)(2)(i): The employer did not establish and enforce work practices for the protection of each worker from the hazards of high-voltage or high-power testing at all test areas, temporary and permanent. Location: 87 West Street. Attleboro MA On 11/06/2025, an employee received a back fed electrical shock while testing a Padmount recloser where work practices were not provided or enforced to identify and control the electrical shock hazard created from the potential transformer tap.
