29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. At an establishment located at 4575 Anston Road, Green Bay, Wisconsin, (a) On or about November 7, 2025, on Line 2 Collator, employees were exposed to potential entanglement hazards when a projecting shaft end on the infeed side of the Collator projected more than one-half the diameter of the shaft. (b) On or about November 7, 2025, on Line 1 Collator, employees were exposed to potential entanglement hazards when a projecting shaft end on the infeed side of the Collator projected more than one-half the diameter of the shaft.

29 CFR 1910.212(a)(1): Machine Guarding. Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are - barrier guards, two-hand tripping devices, electronic safety devices, etc. At an establishment located at 4575 Anston Road, Green Bay, Wisconsin, (a) On or about November 7, 2025, on Line 2, the in-going nip point presented by the horizontal drive belt used to conveyor material from the blue conveyor system to the Collator was not guarded to prevent employees from contacting the in-going nip points. (b) On or about November 7, 2025, on Line 2, the infeed and outfeed side of the R-Pack, Extreme, was not guarded to prevent access to the rotating rollers exposing employees to entanglement and in-going nip point hazards. Louisiana-Pacific Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1790801, citation number 1, item number 1b and was affirmed as a final order on March 19, 2025, with respect to a workplace located at 4575 Anston Road, Green Bay, Wisconsin.

29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. On or about November 7, 2025, at an establishment located at 4575 Anston Road, Green Bay, Wisconsin, on Line 2, while performing maintenance activities on the Rotator Area #2, Zone 3, the employer did not ensure that an authorized employee verified de-energization prior to entering the fenced area exposing the employee to struck by hazards.
