29 CFR  1910.138(a): The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s):  a) At 60 East Lake Road McDonough, GA 30253, MCC Electrical cabinet: On or about 11/18/2025 and times prior, employees performing tasks such as maintenance and troubleshooting within the MCC electrical cabinet were exposed to potential electrical shock and mechanical hazards. The employer failed to provide and require the use of appropriate hand protection.

29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection:  a) At 60 East Lake Road McDonough, GA 30253, MCC Electrical cabinet: On or about 11/18/2025, employees were exposed to the potential release of hazardous energy during tasks such as maintenance and clearing jams in the wash tunnel. The employer used zip ties to secure electrical switches in a presumed "safe" position. These zip ties did not constitute an effective energy control device, as they could be easily removed by any individual at any time, thereby failing to provide positive control over hazardous energy sources.

29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):   a) At 60 East Lake Road McDonough, GA 30253: MCC Electrical cabinet; On or about 11/18/2025, The employer's written lockout/tagout procedures were not specific to the location and failed to meet the minimum performance requirements for controlling hazardous energy. Noted deficiencies included, but were not limited to:  1. Energy Magnitude Not Identified: Procedures did not specify the type and magnitude of hazardous energies present, including electrical, pneumatic, and gravity sources. 2. Lockout Points Not Defined: Procedures failed to identify where locks should be applied to ball valves after switching off compressors. 3. Inaccurate Device Representation: Procedures depicted the use of a conventional lockout device that physically could not fit into the MCC cabinet breaker holes and did not mention the use of zip ties. 4. Incomplete Pneumatic Isolation Instructions: Procedures directed users to turn the conveyor ball valve to shut off pneumatic energy but did not specify where to apply a lock. 5. Lack of Worker Notification: Procedures did not instruct users to notify affected employees prior to initiating lockout/tagout. 6. No Verification Step: Procedures omitted instructions to verify that equipment was in a safe state before beginning servicing or maintenance. 7. Improper Re-energization Instructions: Procedures directed users to simply reverse the steps without providing a clear, step-by-step process for safe re-energization and removal of lockout/tagout devices.
