Hazmat/Waste Area: On or about December 1st, 2025, and at times prior thereto, employee(s) used a Toyota Forklift truck to load/unload bond-rite acetone into large flammable locker(s). The employer did not ensure removal of the power industrial truck(s) that were not in safe operating condition(s). Employee(s) were exposed to struck-by hazard(s).  Following Equipment/Non-Functional Devices:  a) Toyota Forklift Truck, Model No. 8FGU32 (Nonoperational Right-hand brake light)  Notice 1, Item 1, 29 CFR 1910.178(q)(1) as required by 29 CFR 1960.8(b): The employer failed to ensure the  removal of the power industrial truck(s) that were not in safe operating condition(s). The employer was cited for a violation of the same standard on January 28th, 2025, Inspection 1800414, March 21, 2025, of final order/settlement, and means of abatement.

29 CFR  1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage.  Bldg. 468, Bay 13, storage area: On or about December 1st, 2025, and at times prior thereto, the employer did not ensure at least 3 feet in front of the electrical panel was clear. The circuit breaker panel had multiple floor cleaning equipment obstructing the panel. This condition interferes with access and immediate shutdown of a circuit in the panel, particularly in the event of an emergency. This condition potentially exposed employee(s) to electrical shock and/or fire hazard(s).  Notice 1, Item 3, 29 CFR 1910.303(g)(1)(ii) as required by 29 CFR 1960.8(b): The employer did not ensure at least 3 feet in front of the electrical panel was clear. The employer was cited for a violation of the same standard on January 28th, 2025, Inspection 1800414, March 21, 2025, of final order/settlement, and means of abatement.
