One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  Maintenance Shop: On December 8th, 2025 and at times prior thereto, one Dewalt model DWE7480 heavy duty compact job site table saw did not have the manufacturer provided guarding. This condition potentially exposed employees to amputation and struck by hazards

Upper Bar Service Station: On or about December 8, 2025, and at times prior to, the  BB-06  6-Outlet Relocatable power taps (power strips) was series-connected (daisy-chained) to a Commercial Electric 6 Outlet relocatable power tap. These conditions potentially exposed employees to electrical and fire hazards.

The energized electrical outlet was missing the protective faceplate. Employee(s) were potentially exposed to electrical shock and/or fire hazard(s).
