29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a. On or about December 10, 2025, located at 5932 Lavender Field, Lot FRM 226, Grove City, Ohio 43123, the employer failed to ensure that an employee was protected from cave-in hazards while performing residential waterline tie-in work in an open excavation approximately 10 feet deep. Work was being conducted at the north end of the trench without an adequate protective system, exposing the employee to struck-by hazards in the event of a cave-in.
