29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a jobsite located at 7144 Richmond Ave, Kansas City, MO 64133: Employees were exposed to struck by hazards from falling objects and materials while conducting residential roofing activities. Employees were observed doing roofing work at lower levels on the roof without wearing protective helmets while other employees were working at the next higher level on the roof.

29 CFR  1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): 1910.1200(e)(1)  At a jobsite located at 7144 Richmond Ave, Kansas City, MO 64133: Employees were exposed to fire and explosion hazards while conducting roofing activities. The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work. At least one  hazardous chemical used at the site was gasoline. Employees were using the gasoline to power a gas powered air compressor.  Juan Zavala was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(e)(1), which was contained in OSHA inspection number 1793289, citation number 1, item number 1 and was affirmed as a final order on 05/12/2025, with respect to a workplace located at 8415 NE Elmwood Kansas City, MO.
