29 CFR  1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   Enfield Manor-17 Enfield Terrace, Enfield, CT 06082: On December 18, 2025, Employer, did not ensure that Employee's were not exposed to commercial construction fall hazards of approximately 30-feet above ground while conducting roof removal & replacement activities from a 7-12 pitch, steep 30-degree roof without proper fall protection equipment or systems in place.  Tactical Construction Services LLC was previously cited for a violation of this occupational safety and health equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1724862, citation number 1, item number 1, and was affirmed as a final order on June 26, 2024, with respect to a workplace located at 80 Perkins Farm Road, Mystic, CT 06355.

29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  Enfield Manor-17 Enfield Terrace, Enfield, CT 06082: On December 18, 2025, Employer, did not ensure that Employee's were equipped with personal fall arrest systems that could prevent an Employee from free-falling more than 6-Feet down to a lower level.   Tactical Construction Services LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.502(d)(16)(iii), which was contained in OSHA inspection number 1621147, citation number 1, item number 1, and was affirmed as a final order on January 18, 2023, with respect to a workplace located at 1212 Main Street, Hartford, CT 06103.  Tactical Construction Services LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.502(d)(16)(iii), which was contained in OSHA inspection number 1724862, citation number 1, item number 2, and was affirmed as a final order on June 26, 2024, with respect to a workplace located at 80 Perkins Farm Road, Mystic, CT 06355.
