29 CFR  1910.141(a)(5): Enclosed workplace(s) were not so constructed, equipped, and maintained to prevent the entrance or harborage of rodents, insects, or other vermin. A continuing and effective extermination program was not instituted where rodents, insects, or other vermin were detected:  a) Employee break room: On or about January 2, 2026, and at times prior, the employer exposed employees to health hazards, in that the employer failed to establish an effective vermin control plan, ensure every enclosed workplace was constructed to prevent the entrance of rodents and address signs and evidence of rodent activity in areas where employees eat and keep their food and beverages.
