OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees Location: Warehouse On or about January 13, 2026, employees were exposed to struck-by/crushing hazards where compromised uprights on an industrial racking system were observed that significantly reduces the weight-bearing capacity of the rack. A single-point failure can lead to a "domino effect" collapse of the entire row. Bottom portions of the columns of the storage racks were damaged. These columns included but were not limited to, Row E sections: 2, 3, 4, 5, 10, 14, Row D sections: 7, 2, 20, 19, 18, 17, Row B sections: 19, 13, 23, 9, 4, 5, Row A sections: 26, 27, 19, 17, 5, 16, 6, 13, Row C sections: 5, 4, 3
