29 CFR  1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  (a) On or about January 21, 2026, and at times prior, the employer did not ensure that each employee servicing stone cutting equipment in the fabrication shop, including, but not limited to, the wire saw and 3-foot saw, affixed a personal lockout device before working on the equipment. This condition exposed employees to struck-by hazards.

29 CFR  1910.179(b)(5): The rated load of crane(s) was not plainly marked on each side of the crane:  (a) On or about January 21, 2026, and at times prior, the employer did not ensure that the overhead bridge crane on the east side of the stone shop was plainly marked on each side of the crane with the rated load of the crane. This condition exposed employees to struck-by hazards.  Summit Sealants, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.179(b)(5), which was contained in OSHA inspection number 1797905, citation number 1, item number 3b, and was affirmed as a final order on July 18, 2025, with respect to a workplace located at 7329 Reynolds Dr, Sedalia, CO 80135.

29 CFR  1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:  (a) On or about January 21, 2026 and at times prior, the employer did not ensure that steel slings used by employees to lift "boulders" in the stone shop when loading and unloading saws had durable identification indicating the items required by this standard. This condition exposed employees to struck-by hazards.
