29 CFR  1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a- East Facing Side Of Building 16, Meridian Walk @ Princeton Development, West Windsor NJ :  A worker was exposed to fall hazards, in excess of 19 feet  2 inches, when working and walking from a job made wooden scaffold whose walking working surface was not a least 18 inches wide.   Observed on or about 01/15/26.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a - Building 16, Meridian Walk @ Princeton Development, West Windsor NJ :  A worker was exposed to fall hazards, in excess of 19 feet  2 inches, when working and walking from a job made wooden scaffold whose walking working surface was not a least 18 inches wide.  This worker and the company worksite Foreman were not trained nor had they received training by a person qualified in scaffold hazard recognition demonstrating that they both did not understand the procedures to control or minimize hazards associated with scaffolds.  Observed on or about 01/15/26.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level:   a- East Facing Side Of Building 16, Meridian Walk @ Princeton Development, West Windsor NJ:   A worker, walking and working from a improperly erected job made wooden scaffold having a walking/working surface less than 18 inches, was not protected from contacting a lower level nor from free falling more than 6 feet due to the length of their rope lanyard, this rope lanyard containing a shock pack, the rope lanyard's anchor being this rope lanyard wrapped around a sheathed window header and connected back to itself, etc .    Condition observed on or about 01/15/26.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)   a- East Facing Side Of Building 16, Meridian Walk @ Princeton Development, West Windsor NJ:   A worker, walking and working from a improperly erected job made wooden scaffold having a walking/working surface less than 18 inches, was not protected from contacting a lower level nor from free falling more than 6 feet due to the length of their rope lanyard, this rope lanyard containing a shock pack, the rope lanyard's anchor being this rope lanyard wrapped around a sheathed window header and connected back to itself, etc .  This worker and the company worksite Foreman had not received fall protection training as specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  Condition observed on or about 01/15/26.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
