On or about February 5, 2026, at 104 Main St. Funk, Nebraska, the employer exposed employee(s) to electrical shock and fire hazards. Employee(s) were exposed to flexible cords used daisy chained to supply power to equipment including but not limited to portable heaters.

1910.334(a)(2)(i) Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about February 5, 2026, at 104 Main St. Funk, Nebraska, the employer exposed employee(s) to electrical shock and fire hazards. The employer did not ensure that flexible cords used to supply power to equipment were not run through doorways. Flexible cords used to supply power to equipment, including but not limited to portable heaters, were run through doorways .
