On or about 3/2/2026, an employee in a basket on the forks of a powered industrial truck (PIT) was not protected from falling by means of guardrails or a personal fall arrest system. An employee was not wearing a personal fall arrest harness or belt and fell from the powered industrial truck to the floor below.
