29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) New single-family residential structure located on lot #23 at 176 Classic Overlook Road Homer, GA 30547/Front of structure - On or about 03/05/2026, the employer exposed employee to the fall hazard of approximately 10-feet 9-inches to the lower ground below, in that the portable extension ladder used to access the upper working area did not have a grasping device to assist employees in accessing the upper landing area.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. a) New single-family residential construction located on lot #23 at 176 Classic Overlook Homer, GA 30547/Roof - On or about 03/05/2026, and at times prior, the employer exposed employees to struck-by hazards in that, the employer failed to ensure that protective eyewear was provided to employees engaged in framing activities while operating pneumatic nail guns. b) New single-family residential construction located on lot #24 at 176 Classic Overlook Homer, GA 30547/Roof and second floor - On or about 03/05/2026, and at times prior, the employer exposed employees to struck-by hazards in that, the employer failed to ensure that protective eyewear was provided to employees engaged in framing activities while operating pneumatic nail guns and circular saws. Alejandro Juarez Perez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1820322, citation number 1, item number 1 and was affirmed as a final order on 08/14/2025, with respect to a workplace located at Lot #8A at 128 Classic Overlook, Homer, GA 30547 .
