An employee was attempting to dislodge a stuck part on a roll forming machine when his right hand was caught between two pieces of metal and the machine guide, amputating his little finger around the middle knuckle, damaging his ring finger, and cutting his middle finger (requiring stitches). The machine was guarded at the time.
