On 7/13/2018, an employee climbed an 8-foot A-frame ladder to tape an area for painting. The employee finished and was starting to descend when the employee missed one of the ladder rungs and fell to the ground. The employee suffered a head cut and broken right ankle, requiring hospitalization and surgery.
