Employees were troubleshooting a triethylene glycol (TEG) pump on an ethylene glycol vessel system. After shutting down the pump, employees closed the valve on the system. Pressure built up and blew holes in the vessels (through the walls of the pipe). The employees were sprayed by hot liquid glycol and one employee sustained burns.
