On September 12, 2024, an employee was standing halfway up an 8-foot ladder while installing a 14-inch spiral duct through a wall. While being pulled from the wall, the duct gave way and knocked into the ladder, causing the employee to fall to the floor and be impacted by a hammer located in his toolbelt. The employee suffered bleeding in his left kidney, two broken ribs, and a laceration to his left temple.

An employee was unloading freight from a truck when a stack of doors fell over and knocked the employee to the ground. The employee sustained a fractured femur.

The employee's cheek made contact with a connector with damaged insulation. The employee was shocked, briefly lost consciousness, and fell to the ground, suffering an injury to the left shoulder.
