Final narrative
Two employees were in a drive terminal, pulling a new fuel line for an EPU to provide tertiary power for a ski lift. After unspooling the fuel line, one employee took a step backward and fell through the skinning (the floor of the terminal) to the ground about 12-15 feet below. The employee suffered a dislocated ankle and fractures to the lower tibia and fibula.

An employee was conducting pressure washing on the roof of a commercial building. The employee fell through a fiberglass skylight panel and struck the concrete pavement approximately 30 feet below. The employee sustained fractures to their pelvis, femur, and heel. The employee was using fall protection at the time of the incident, including a harness and lanyard, but it failed to arrest the fall.
Titan Carpentry, LLC
Employees were installing wood roof trusses, setting them on concrete masonry unit walls that were capped with new double 2-by-6 top plate. The trusses collapsed in a chain reaction, causing three employees to fall to the slab below. One of them, who had fallen about 20 feet to the concrete, suffered two collapsed lungs, several broken ribs, and a broken pelvis. The employee was hospitalized.

On August 20, 2025, an employee was working from a scaffold while painting the inside of a 30-by-30-foot ground-level water tank. While descending the scaffold, a wheel on the brace broke and the scaffold tipped over. The employee fell approximately 18 feet to the ground and suffered a broken pelvis.

An employee was working on a second-level roof area. He jumped down to a porch roof, fell through it, and struck the ground. He suffered broken bones and was hospitalized.

An employee was carrying material on a flat roof, about 19 feet high. He tripped and fell through an area of corroded roof decking, landing on the concrete floor below and suffering a fractured pelvis and skull.
