Employee was using utility knife to cut score plastic sheet to allow treater electrode to light up and treat the plastic sheet. Employee was wearing cut resistant gloves and sleeves ear plugs safety glasses. and steel toe shoes.

Employee stated that while cutting scoring sheet thought that they hit something with the utility knife causing them to get shocked electrocuted. Received burns to hand and foot.

Electrical burns to hand and foot. Burn on foot approximately 16 days after event was not healing properly and doctor wrote employee off for wound care. Initially wrote off work beginning on February 15th; returned on March 12th. Had healing issues on foot and wrote off work again beginning on 9 20

Electrical shock and burns from Treater unit.

Shock electrocution burns to right hand(palm) and left foot. Wrote off work 2 15 returned 3 12 went back off work 9 20 for wound care on foot.
