What happened
Employee #1 was unloading a trailer and placing the pallets of ingredients onto the Warehouse Conveyor. Employee #2 was pulling the shrink wrap up on one side exposing the bottom bag of ingredients so they could place a label on the bag. While reaching down to pull the shrink wrap up it was wrapped to the bottom of the pallet so the employee was trying to pull it from under the pallet as the pallet was rolling down the conveyor. As he reached under the pallet to grab the shrink wrap his right ring finger was caught under the pallet between the pallet and the roller.
