Please see answer to Question 15 below for details.

When completing a changeover they stop sending pans for the dough as quickly as possible however the few pans that do go through have to be removed manually from the line because sending empty pans through the line causes damage to the equipment and the pans. The employee was standing at the exit of the bread moulder and was double stacking pans so the other employee further down the line could remove 2 at a time to stack onto the pan cart. The employee was grasping the ends of the pans to pick them up and when they did this a conveyor stopped moving and the pan coming out of the bread moulder slammed into their left hand and pushed the pan into the pan that was waiting to go on the next conveyor which also jammed their right hand.
