Before the incident

Employee was working in blender room reinstalling a coupling on a resin pipe A small section of sheet metal grounding strap was sticking out of another resin pipe (next to the one employee was attempting to reattach) When attempting to realign the bottom pipe the employee made contact with the grounding strap piece and lacerated his ring finger on his left hand This was a nonroutine task as these couplings do not typically need to be removed The coupling was removed due to water in the resin line The water was a result of a broken fire suppression pipe in the room caused by the below freezing temperature in the facility
