EE was reporting to work in Lamination building and experienced a ground level fall walking from the parking lot The EE complained of some tenderness soreness posterior lower back and struck the back of the EEs head EHS followed up with the EE as they worked and at the time the EE felt fine to work 3 6 EHS observed the EE moving slow and protected as they preparing material for the feed table of [REDACTED] Further discussion with department Lead the EE elected to go home 3 7 The EE called Medcor on 3 7 and urged to seek medical attention EE visited Summit Urgent provided prescriptions and released to RTW immediately with less than 10 lifting restriction no twisting bending crawling 3 13 EE was removed from Restrictions on 3 13 without further complaints
