Before the incident

[REDACTED] around [REDACTED] an employee was joining heads on 7 Moulder and as the machine spun down it went unnoticed that the bottom head had not come to a stop yet Other heads had come to a complete stop The employee ran a piece of wood across table and fencing to position the hold down plate in place when the employees left middle ring and small finger contacted the still spinning head resulting in significant lacerations to two of the three fingers EE was taken to an Urgent Care where the EE received six sutures in the Left ring finger and splints to the middle and ring fingers secondary to fracture to the distal phalanges EE will return to work on [REDACTED] with modified duty 2 28 3 1 EE elected not to RTW secondary to soreness and the pain medication 3 4 EE elected to quit 3 28 EE signed and DECLINED the offer letter for modified duty See attachment
