EE reached for a Klever Kutter cutting tool to remove plastic wrap banding from material where the cutting tool had been hung by a sting When the EE pulled the cutting tool towards the work the string attached did not clear its retaining hook which pulled the cutting tool through the EE Right hand As a result a laceration to the EE middle finger on R hand The EE brought it to the attention of a Lead who encouraged him to seek medical attention No notification to Department Manager or EHS at the time The EE went to urgent care and then give sutures to R middle finger The EE had previously scheduled PTO for 3 4 3 5 upon returning on [REDACTED] gave the Manager the 827 form The Klever Kutter shoulder that covers the blade was broken leaving the blade partially exposed
