Before the incident
Taking scrap counterweight racks to scrap metal roll off dumpsters
What happened
Employee was moving scrap steel to the roll off dumpsters on the NE corner of the property and got too close to a trench that is under construction for the tank farm construction project.As a result the back tire of the forklift caught the open trench causing the forklift to tip into the trench.The operator of the forklift sustained moderate injuries but was able to get out of the lift on their own.Employee was taken to the Emergency Room to be evaluated for the injuries.
