The ironworker attached his fall protection (Cable Cheater Leading Edge SRL Harness) to the iron beam that he connected.

He made his way to the rigging point to disconnect the beam he had connected from the crane. As he was walking the beam to get to the rigging The ironworker was pulled backward by his SRL which caused him to lose his balance and fall off the beam he was walking. Due to his connection point inside the aerial lift basket the fall caused a pendulum effect and caused him to swing into the bottom of the boom lift basket twice. The first time he knocked his hard hat off his head and the second time he caused a laceration to the top left side of his head. The employee pulled himself back onto the iron and reported the incident with help from two fellow ironworkers. He got off the iron and was taken to the GC office to get checked out. The ironworker never disconnected his SRL from the anchor point inside the basket so he was pulled backward.
