What happened
Ironworker was picking up a piece of a flat plate 1” thick 4” wide 8 ft long. He lifted at the halfway mark of the piece bending his knees and lifting. When he reached a full stance the piece was unlevel on his shoulders and slipped out of his hands sliding off his shoulders and falling to the ground. He moved his left foot out of the way but it fell onto the top part of his right foot.
