A Safe Work Permit and Line Break Permit was issued for the removal of two valves and multiple gasket replacements on the HF Feed System in [REDACTED] located on the east side of the 5th deck The initial task was carried out in fresh air and while wearing acid suits with operations witnessing the first two line breaks After completing the initial two line breaks the system was open and did not show signs of having product in line The Line Break Permit was then returned to operations The workers proceeded with the tasks outlined in the General Safe Work Permit which included replacing several gaskets within the piping system While loosening bolts on 1 inch flanges to replace old gaskets hydrofluoric acid liquid and vapors were released from the line contacting the injured employees face
