[REDACTED] SWS employees were preparing to make a cut on a 6 inch piping system in an elevated area Employees were required to climb a fixed ladder to a permanent deck where they would transfer to a scaffold ladder to reach the work area The fixed ladder is approximately 10 feet from grad to the work platform As an employee was ascending the ladder he lost grip causing him to fall approximately 8 feet to the ground As a result of the fall the employee experienced a fracture to the left hip area requiring surgical intervention to repair
