What happened
Left palm puncture The employee was feeding wire into the draw die. He had clamped the wire down with the holding block and started to pull the wire when the wire broke. As he was pushing the wire back into the block he pushed it with the broken part of wire it then came out the other side and punctured the palm of his left hand.
